Are You at Risk?

Risk factors are conditions or habits that make a person more likely to develop a disease or increase the chance that an existing disease will get worse.

Are you at risk heart questionnaire.

Check all statements that apply to you:
  • I am a man over 40 or a woman over 55 or younger and post-menopausal.
  • I have a family history of heart disease.
  • I have high blood pressure.
  • My total cholesterol is 200 or higher, or I don't know my cholesterol level.
  • I smoke and/or live or work with people who smoke regularly.
  • I get less than 30 minutes of physical activity at least three times a week.
  • I am 20 pounds or more overweight.
  • I have diabetes or pre-diabetes, or take medicine to control my blood sugar.
  • I have a strong family history of heart disease.

If you answered yes to one or more of these statements, you are likely at risk for developing heart disease. The more 'yes' statements means the higher your risk. The first step toward heart health is becoming aware of your own personal risk for heart disease.
